I'm not sure if this is the right spot for this post. I figured someone might have some experience at this in Indiana.

I was a piece worker for a company in Maryland doing mostly carpet and now I moved to Indiana. I have always worked as a piece worker for a company. Now I need to find work as a sub or just do my own work. 

My question is: Does anybody know the process of becoming a flooring sub contractor or an independent contractor in Indiana?

I know I need insurance but I don't know if I need any licenses or if I need to start an LLC to operate.

Trying to operate legally, not everybody likes to pay in cash. haha

You should call the stae or local contracting licensing board.
you can also register a company name as a DBA, this will allow you to open a bank account in the company name and accept checks for payment,
forming a LLC can be both beneficial and a nuisance, if you are married, put the company in your wifes name and have her pay you through a payroll service each week, this will allow her to pay for un-employement insurance and also for workmans comp, that you can collect on if your injured, You cannot legally collect on WC insurance by insuring yourself. General liability runs somewhere around 3 to5 hundred for standard policy to cover damage etc.
hope this helps out some, best bet is to contact the state tho to make sure you comply with all regs
