Granted, there appears to be evidence of another object striking the floor due to the color of another object left behind. Looking at the fractures on the left/bottom left was there not a void?

What is up with the color to the right of the coin?
Who ever grouted that is a pro fer sure, perfect. :)

The upper portion of the hole has a slight void if you can see the slight shadow. Of course there is going to be some dirt and stuff around from mopping but could that dark spot be some kinda foreign matter that caused the void?

Dan, the color to the right of the coin is just a lower part of the tile surface thats supposed to mimic slate.

So if a fracture how could the edge be elevated and not just a chunk taken out. Beats me.
sure looks like impact, think i'd wanna take samples of the black stuff across the top and whatever the red scrape is. CYA my friend. victorian chest of drawers or dresser? odd shape foot anyway. the red is your clue.

Nice Sacajawea $1 piece, used to use them to tip the cute waitresses, anyhow, is this ceramic? If so it looks to me like a hollow flaw inclusion was present, and then impacted with something later, too much of a hole if the material is hard at all.
Probably what Selva said. There's no grout down in it so it must have occurred later. A hammer can cause that too, but It's very possible a small defect in the tile that just popped.
Yep it's ceramic Leo. I never heard of pop outs on ceramic, just Satillio tile. This is in my house. Just appeared one day but it's by the fridge which is suspect!

Still can't see how an impact can cause that since there is a lip sticking out over it on the top, hollow underneath. Never seen an impact like this one.
Well, if it were a popped tile you should have seen a chipped piece near it. If you did not the I suspect someone dropped something and is afraid to tell ol'Dad ;)
Looks like it caved in, not really a crater like event.
I think that coin can be removed with a putty knife.
Ha, not a bad idea, bet that is good for a few laughs from the locals. Reminds me of my grandfather who used a casting process to form a mold from original silver dollars and recast fake lead coins. He then use a brace and bit to countersink five holes into his limestone sidewalk and drop in fake silver dollars. It was covered with an old rubber entryway mat and if he took a liking to his present company, he would "unveil" the treasure they had walked across. Onery old guy had many a laughs listening to people tell him he should remove them before someone steals them.
